Beginning March 3, 2020, laboratory operations have ceased and tests and services will no longer be offered. The ordering physician will route the test request to his or her preferred testing laboratory.

Medical Records Request and Fixed Specimen Materials (slides and blocks):
Medical records and fixed specimen materials for current and past patients will be retained in accordance with the State of California Business and Profession Code 1295, Clinical Laboratory Improvement Amendments (CLIA) 88, and College of American Pathology medical information preservation, and specimen materials retention guidelines. Medical Records and fixed specimen materials are protected by HIPAA and could only be accessed by formal written request. After the date of closure, the responsibility for maintenance of medical records and fixed specimen materials retention will be transferred to IMD Path which will act as custodian of those records and fixed specimen materials. After March 3, 2020, former patients and physician clients can request medical records, and access to fixed specimen materials created at PathLogic through these methods:

We cherished our relationship with patients, community physicians and health care team members that we have served over the years. Unfortunately, PathLogic could not endure recent difficult economic times in healthcare and despite the outstanding staff, state of the art facility and service delivery system, it remained unsustainable.
